If the body part is masculine, use au
J'ai mal au genou: my knee hurts
If the body part is feminine, use à
J'ai mal à la jambe: my leg hurts
If the body part starts with a vowel, use l'
J'ai mal à l'orteil : my big toe hurts
For the plural, use aux
J'ai mal aux yeux: my eyes hurt
J'ai mal aux oreilles: my ears hurt
